UPMC Registered Nurse Casual in Lemont Furnace, Pennsylvania

UPMC Home Health is hiring a Casual RN to

cover Fayette and Westmoreland Counties

This is a daylight position with various shifts between 8am-4:30pm

The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes according to the physician's plan of care, agency policies and procedures, fiscal intermediaries, regulatory agencies and bodies, and within recognized standards of nursing practice. Must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to the population served. Establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. Assignments will be determined by the Clinical Manager of the branch based on several different factors, which include, but are not limited to, coverage for Leave of Absence, an increase in patient volume in a specific team, and vacancies.

Responsibilities:

  • Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patients. Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Completes an initial and comprehensive assessment of the patient.

  • Constructively receives feedback and direction. Takes action to improve knowledge, skills, and performance based on feedback or self-identified developmental needs. Initiates, coordinates, and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family-centered health teaching. Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members by the physician's orders and patient needs.

  • Assign other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient. Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed. Attends and participates in case conferences, team meetings, and other work-related meetings. Recognizes limitations and requests assistance and direction to meet performance goals. Completes all necessary documentation which may include visit assessment, plan of treatment, OASIS, 485, and Verbal Orders accurately and promptly in the electronic documentation system while in the patient's home and within the agencies and regulatory standards. Data syncs (transmits) information same day.

  • Identifies, investigates, and recommends solutions to organizational issues. Exercises independent judgment in matters concerning emergent and non-emergent patient care needs and communicates with the physician. Identifies, develops, and participates in process improvement opportunities within the agency that will enhance the quality of the services we provide. Participates in weekend and on-call coverage rotation for the agency as per BU guidelines. Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, including complex and/or specialized care.

  • Performs in accordance with system-wide competencies/behaviors.Performs other duties as assigned.

  • Zero to two years of experience.

  • 6 months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.

  • BSN preferred.

Licensure, Certifications, and Clearances:

Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Graduate nurses must complete the licensure examination within FOUR MONTHS of hire or one year of graduation, whichever comes first. Though temporary permits are valid for one year from the date of graduation, not one year from issue of permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 4 months of their start date, or they will be terminated or demoted from the GN position, at the discretion of the BU.
