UPMC Social Worker - MSW-UPMC Kline Health Center in Harrisburg, Pennsylvania
The Kline Health Center is looking for Social Worker to join!
This position requires traveling with home visits and communities.
This position is Monday-Friday 8:00-4:30P, no weekends/holidays.
Purpose:
The Social Worker is responsible for providing complete psychosocial assessments, crisis intervention, individual, family, group, bereavement and end of life counseling, and timely and appropriate discharge planning based on known or anticipated needs.
Responsibilities:
Needs assessments, provide resources to patients and staff, re-ordering Pfizer medications, DME Referral process, DME Referral follow-ups.
Care Coordination, Team Collaboration & Communication, UGI, PPL and Capital Region Water Medical Certifications.
Pfizer Patient Assistance Program Initial and Renewal Applications (as well as other possible company patient assistance program applications).
Scheduling transportation, Home Visits, Waiver Referrals, assisting community agencies and/or patients in getting forms completed by providers.
Providing counseling regarding options and resources and advocate to providers and/or agencies on the patient's behalf.
Participate in Kline Care Coordination and Home Visit Care Coordination, weekly meetings, ensure referrals, medication vouchers, taxi vouchers.
Managing uninsured population-i.e. referring to UPMC Pinnacle Enrollment Specialists, HRSI and/or APPRISE Counselor & putting a plan in place.
Manage Kline Resource Center with Community Initiative SW, Participate in trainings/continuing education.
Monthly SW OP Meetings with different community agency speakers/Networking.
Participate in meetings with community agencies to introduce/educate Home Visit Program.
Created Community Resources book and DME Resource book Kline Staff/Residents.
MSW degree from a CSWE accredited School of Social Work required.
Licensure, Certifications, and Clearances:
Current / active Social Work License required or successful pass of the social work licensing exam within 9 months of employment.
Clearances must be dated within 90 days.
Clinical Social Worker (CSW) OR Licensed Social Worker (LSW)
