UPMC Staff Pharmacist, UPMC Altoona - Sign-on Bonus Available! in Altoona, Pennsylvania

We are offering a $15,000 sign-on bonus for a 2-year commitment or a $30,000 sign-on bonus for a 3-year commitment!

Purpose:

Fills prescriptions, compounds various preparations, performs sterile IV admixture techniques, provides drug information, serves on various Hospital committees, gives in-services, trains and instructs new employees, students, and patients, supervises Pharmacy support personnel, and provides for quality Pharmacy practice.

Responsibilities:

  • Monitors drug therapy for contraindications, drug-drug, drug-food interactions, duplication of drug therapy, allergies, appropriateness of drug, dose, and interval of administration and recommends changes.

  • Participates in Continuous Performance improvement and medication use review activities of the department. Collects data; conducts quality monitors, maintains logs, records, and other documentation as assigned.

  • Supervises and directs Pharmacy support personnel, and verifies the daily activities of the Pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel.

  • Able to perform all required Pharmacist level functions on all Pharmacy Information Management Systems.

  • Provide pharmaceutical information to Medical Staff, Nursing Staff, etc. Provides clinical pharmacy consultation and clarification to clinicians. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Reviews /interprets culture and susceptibility data for appropriateness of antibiotic selections and recommends changes.

  • Correctly interprets and enters Physicians' orders. Correctly assess each patient's medication requirements based on Diagnosis, labs, and current medications ordered. Correctly fills and /or reviews filled medication orders.

  • Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided promptly.

  • Provides pharmaceutical care to all ages of patients neonates to the elderly. Recognizes when the dose, frequency, and dosage form must be adjusted for these different age groups and recommends changes. Provides pharmacokinetic consult services and drug dosing as per protocol. Provides drug information to patients and their families. Provides discharge medication counseling when required.

  • Participates in the development and presentation of orientation and training programs to the Pharmacy, Medical, Nursing Staff, and others.

  • Supports the organization's formulary by minimizing the use of non-formulary agents, recommending formulary alternatives, utilizing therapeutic interchanges, and promoting rational evidence-based drug therapy.

  • Accurately prepared or checked compounded preparations. Accurately /aseptically prepares and/or inspects sterile Products. Accurately fills and receives controlled substances and makes correct documentation of all transactions via C-II Safe.

  • Identifies and reports suspected adverse drug reactions as outlined in the SOP. Documents all clinical interventions accurately and completely.

  • B.S. degree in Pharmacy or Doctor of Pharmacy (Pharm D.) required.

  • Maintain a License in good standing as a registered Pharmacist in the Commonwealth of Pennsylvania.

  • Prior experience as a Pharmacist in a health systems practice setting is preferred but not required. Licensure, Certifications, and Clearances:
